***Note: As building renovations are expected to last throughout the summer, registration logistics are still being formulated. More details will be shared here and on our socials when available.***

2026-2027 Registration Forms
Please download/print the forms below then bring all documents to Mosaic for further processing. Forms are also available in the main office. CPS currently does not accept nor process applications electronically. Any further questions regarding registration can be directed to our clerk, Ms. Claudia Ortiz ([email protected]).
 
Registration Requirements
  • Students are 5 years old on or before September 1st, 2026
  • Must live in the neighborhood attendance boundaries
  • Check your address on CPS's School Locator tool

Other documents are required for enrollment by our main office:
  • Proof of age document (birth certificate only)
  • Two proofs of address documents (examples: gas/electric bill, Illinois driver's license or state ID card, deed, etc)
 
Pre-K Process
PreK enrollment is different than what's listed above for all CPS schools. Children must be three or four years old, but not yet five years old, on or before September 1st, in order to be age eligible for a CPS preschool program. Enrollment in any CPS preschool program is subject to availability of space. Please contact Chicago Early Learning at 312-229-1690 or visit chicagoearlylearning.org
 
Homeschooling
CPS also offers homeschooling options. As of January 6th, 2023, registration forms will not be accepted through mail or email. Please visit the official CPS site for more information. Any questions related to homeschooling should be directed to [email protected]
